Coleman Nature Kit magnifying glass. I found two large button batteries in the toy box and they belonged to the magnifying glass. Upon inspection, the door to the batteries is dangerously designed and it opened on its own. Luckily my toddler hadn't taken to the batteries and I disposed of them.
Consumer stated that the circular rim of the helping magnifier he purchased is very sharp. The consumer has bought 3 of these products from the retailer. The consumer's daughter cut her right index finger when she tried to remove the protective plastic from the magnifier, resulting in a her applying a band aid. She did not go to the doctor nor seek medical attention. The product is a manual tool. The incident occurred as soon he presented product to his daughter and she tried to open the product. The consumer has yet to call the retailer about the product but does intend to return two of the magnifiers. The consumer has yet to call the manufacturer about this incident. Update: Consumer returned all 3 back to the recently opened Braintree store on Aug 09, 2011 (store #351) (Copy of return receipt enclosed)
[REDACTED] I was at a restaurant when a woman at the next table turned on a device that was about the size of a cell phone, but had a ring of intense, blue-rich LED lights, acting something like a flashlight. I was struck directly by the LED lights, and a few seconds later began going into a panic and meltdown. I got up from the table and began breathing hard and made my way outside, and then collapsed to my knees and began crying.
While using a Bushnell Sportview T1403 432 riflescope mounted on a [REDACTED]-branded .30-06 bolt-action rifle, a safety-related failure occurred. The small metallic turret indicator disc (part of the windage adjustment dial) detached from the scope and fell into the action of the rifle. The disc became lodged in the bolt and chamber area, jamming the rifles bolt. It was crushed during bolt operation. Although no injury occurred in this case, I believe this incident presents a serious safety hazard due to the nature of how the disc entered the firearms action. If a shooter were to unknowingly force the bolt closed with the disc obstructing the chamber, it could lead to: An out-of-battery discharge, Gas or metal blowback toward the shooter, Catastrophic failure of the firearm, and Potential injury or death. This part failure occurred without abuse or modification. Photos of the damaged scope and the bent disc are available and were submitted to Bushnell Customer Support for review. I am submitting this report to raise awareness and in hopes of initiating a safety evaluation of this failure mode.
I have a small magnifying glass with a light that allows you to see things up close The light had stopped working. I opened the battery case on the magnifier and there were three batteries inside, fitted tightly, that looked similar to hearing aid batteries. I had to pry the batteries out of the compartment with a small metal flathead screwdriver. The batteries were dumped onto a couch. moments latter, one of the batteries exploded, sending small mental shards through the living room. No injuries were sustained. I was wearing glasses and I felt some of the debris hit my glasses. Without glasses, I suspect I would have had an eye injury. The magnifying glass was purchased at Barnes & Noble Book Store several years ago and no receipt exists. The "brand" name on the magnifier is "Mighty Bright" and on the back a stencil saying, "Made in China".
We bought a pair of kid binoculars at the Atlanta Zoo gift shop. They are small and definitely for children. My child was swinging outside and came in with a serious mark around his neck. He had tried to get off the swing but the binocular string had tangled and was choking him. Thankfully the string broke at the plastic point where it was connected to the binoculars. It did not have a break-away clasp at all and I didn't think about it. It seems that since these are being marketed to children they should have a safety break-away clasp. There is no identifying information on the binoculars, it just says "Zoo Atlanta".
Five year old Grandchild received a Bebang handheld microscope for kids labeled by them on the Amazon product page for ages 4+. It included glass slides. The slides were very thin glass and had no covering or reinforcement along the edges which were not smooth. (This is a significant difference from the product which they were clearly copying to sell for a lower price.) The slide broke into two pieces during play and the edges created were very sharp. Child was cut by it and needed basic first aid. I am concerned that this product may not have gone through compliance testing and will be dangerous to others. I was able to quickly find a reviewer whose child was cut during use in the Amazon reviews.
My 5 year old got these binoculars for his birthday and his 3 (almost 4) year old brother was playing with them in the next room. Within a couple hours came frantically in the room with the cord wrapped around his neck tightly several times and the binoculars stuck behind his back and couldnt reach them. The safety latches were not releasing properly and I fought with trying to untangle the several tight twists.
