We have had a Schlage Encode Smart Wi-Fi Deadbolt (with Century Trim in Satin Nickel) electronic lock on our home's front door since around January 2024. This allows us to use a code to enter our door (so we don't have to carry a key). After a couple months, we precipitously had an issue with the electronics on the first lock (with no warning, it just stopped turning on, even when the batteries were replaced, but we could still manually turn the deadbolt, so it was a pain, but we could still get in and out of the door). We contacted Schlage and they sent us a replacement lock. It seemed like the new lock was working okay until today, again out of nowhere, we we unable to manually unlock the door. The deadbolt would not undo from the inside. All adults in the household tried. We were locked in. Thank goodness we have other doors in the household, and thank goodness my husband was present and knew how to uninstall the entire lock. Once he removed all electronic aspects of the lock and was down to just the deadbolt (with the hole for the locking mechanism open between the outside and the inside), he was able to manually unlock the door with a tool. If we have been trying to exit in an emergency, particularly a fire, we could have all died because of this serious malfunction. We are going to remove this electronic lock and return to a basic locking mechanism, but it's important that this major safety concern be known.

My Kwikset 99380-001 Halo Wi-Fi Smart Lock Keyless Entry Electronic Keypad Deadbolt melted and burned the tips of two of my fingers on Sept 23, 2021. On the late evening of Sept 22, 2021 I was in the laundry room that has a door to the garage and I thought I smelled a faint smell of something odd, like a sort of burnt toast. I checked all around and could not find the smell and it went away. The following day the Kwikset door lock in this room alerted us that its battery was dead and needed replacing. Later on Sept 23, 2021 I removed the battery pack from the lock housing and replaced all three 1.5 volt AA batteries, then reinstalled the battery pack . I then went to verify the locks operation and the lock would illuminate but would not lock or unlock. Then I started to smell a strong smell of something burning and I could see smoke emanating from the just installed battery pack. I tried to remove the battery pack from the lock housing by lifting up on the plastic removal tab, as I had done previously, it is at this point I burnt my finger tips on the molten plastic. I then went to the garage and got a dental pick to pull the battery pack out. Once the battery pack was removed the battery pack stopped melting and smoking. My fingers were sore but OK, no lasting damage. I then notified Kwikset about what just happened and uploaded pictures to them using their online tool. I asked Kwikset if there were any recalls on this product and they said no and there were no know issues. They issued me a case number and said they wanted the complete assembly returned to them for analysis using the prepaid label they provided. They told me they were sending me a identical replacement as soon as they reviewed the pictures. I am concerned for several reasons: - My fingers got burned. - I do not know what would have happened if I was not home and this happened as the door to the garage is a light wood door. I have purchased many "Safety 1st OutSmart Flex Lock" and after some number of months they all fail. A plastic internal "catch" used to secure the lock breaks off, which could be eaten by a child, and the lock becomes functionally useless. It does not take pulling on, misuse, etc. to cause this - seemingly just age, as if the internal component is dry rotting. As this product is most likely to be used to secure cabinets beneath a kitchen sink where chemicals are typically stored, it seems highly probable a child will die from the sale of this product. I've reached out to the manufacturer, but thought this should be reported here as well.

I was trying to troubleshoot my cousin’s EUFY smart deadbolt lock which had gotten stuck. While researching solutions online the official customer forum results showed a large number of people complaining that the deadbolt gets stuck in numerous scenarios. Most dangerously, according to reports the lock gets stuck in a locked position and cannot be unlocked manually from the inside. Here’s a link [REDACTED] This is clearly a product defect, not an installation error, as those online reports mention that Bluetooth or app unlocking still works. Of course, in a fire this presents a huge danger as one firefighter in the customer forum points out. Even though a customer filed a complaint in the official Eufy forum nearly nine months ago (and other customers complained of the same dangerous issues), the company has provided no official guidance or troubleshooting steps. Customers also do not seem to have identified a DIY solution.

I purchased the Tumalo Brass Knob Exterior Door Hardware Set from Rejuvenation in August 2019, and since then, I have encountered multiple safety-related issues with the product. These issues significantly compromise the security and safety of my home. 1. First Issue (February 2022): After about 2.5 years of use, the deadbolt mechanism became extremely loose. The locking mechanism started failing intermittently, requiring frequent adjustment and posing a security risk. I contacted Rejuvenation at the time, and while they acknowledged that this should not be happening, they only suggested that we attempt to fix it ourselves by tightening the screws. 2. Second Issue (August 2024): The interior doorknob completely detached from the spindle, rendering the lock entirely inoperative. Upon inspection, I found that the spindle had split in half. This is a serious concern, as the inability to securely lock the door puts my household at risk of break-ins. Rejuvenation declined to offer a replacement part or meaningful solution, citing that the product was outside of its warranty period. The failure of both the deadbolt and the doorknob within five years poses a serious safety hazard. An unreliable locking mechanism leaves the door vulnerable to forced entry, which could lead to personal injury or property theft. Given the high price of the product and its intended use for home security, this level of failure is unacceptable and dangerous.

I own a home with a storm shelter (FEMA rated Safe Room) built in. Is is made of filled concrete block with a concrete slab on the top and bottom and a FEMA rated storm shelter door as the only exit. The door has 3 Medico single cylinder deadbolts and an ASSA passage set for entry and exit. Yesterday, I found myself locked out of our storm shelter and determined that the latch was not retracting when the handle was turned. Having had locksmith training, I knew the problem was that the latch had become disengaged from the mechanism that retracts the latch when the handle is turned. The only way to get the door open was to remove the handle and bezel and cut off the outside flange with an angle grinder since the hardware is only designed to be removed from the inside. Unfortunately, the paint on the door was damaged a little in the process and the lock was destroyed. After removing the lockset and getting the door open I was able to confirm the problem. When the latch was installed it was not completely perpendicular to the lock mechanism; it was slanted slightly toward the exterior of the door. The lockset's retraction mechanism has a slot into which the latch retractor fits. There is a gap on both ends just wide enough for the retractor to slip out of contact with the retractor. The latch retractor was able to work its way into the gap on the exterior side so the little ears on the retractor were not within the slot. When the handle awas turned it failed to pull the latch with it, effectively locking the door even though it is a passage set. I do not know why ASSA would leave a gap like that in the retraction mechanism. Making the tab thicker on the latch retractor would prevent this from being able to happen. The passage set worked fine for the last year or so; it did not happen right away. This is a serious problem. Adams Rite Model 4300 SteelHawk failure of internal screw or brass roller mechanism causes lock to become inoperable from inside or outside. This will cause entrapment as door will not release with inside trim or outside trim. We have 7 of them deployed in the field and all have had to replaced at least once for the same issue. This is extra dangerous as these are designed to be used as a card reader or electrically controlled lockset on the front a public business. There is no manual override of the locking latchbolt when this part fails.

I'm am writing you regarding a safety concern I've become aware of with the gun locks provided through the Project ChildSafe program. As I understand it, the City of Canyon Lake, California received a donation of the Project Child Safe gun locks through the Riverside County Sheriff's Office. A portion of these locks were then donated to the [REDACTED]. These locks were then intended to be purposed to assist with the Quagga Mussel Inspection Program as a means to physically lock a contaminated boat to its trailer to prevent the boat from launching within the lake. Upon inspection of the approximately 60 locks we received it was discovered that a large amount of these locks were malfunctioning and would open without the use of the key. A small amount of vibration or tapping on the frame of the lock would cause the locking mechanism to jar loose and the cable to run free. My concerns do not lie in the area of what the locks were now intending to be used for, but for the original purpose of securing a firearm from endangering a child. I currently serve as [REDACTED] and I have extensive training in firearms safety and responsibility. I own several guns which I store responsibly in my home to prevent my children from accessing them as required by law. I believe that proper education is the first and foremost deterrent to children improperly handling firearms, and that a dependable means of locking the weapon is critical to ensuring this. After seeing 1 in 5 of these locks fail to maintain a secure connection, I would never trust nor allow any of my firearms to be secured with these locks. I am urging you to take a hard look into this claim to find the cause of the malfunction. I think the purpose of this program is commendable and worthwhile, I would regret for the reputations of the agencies involved to become tarnished by just one of these gun locks failing in the most unfortunate of circumstances.
