During a 6 year teardown of Ansul Sentry 5# ABC Portable Fire Extinguisher, model #AA05-1 it was discovered that the siphon tube used in expelling of chemical dry powder was not long enough to fully discharge extinguisher. There were 4 extinguishers with this model number and 2 had siphon tubes of inadequate length. All extinguishers looked at are manufactured in 2011.
I don't have an incident, but I came across this on Amazon and it made me concerned. The fire extinguisher is listed as being >99% water-based, but also as certified for class B, C, and K fires. My understand was that water is never appropriate for C or K. I've just realized that I didn't include the Amazon listing. It is here: https://www.amazon.com/dp/B0B586JB7M/
Aksipo Fire Blanket Fiberglass Fire Emergency Blanket - when I opened it fiberglass went EVERYWHERE, into my skin (possibly eyes). Took hours to clean up.
During a brush fire, this Kidde Fire extinguisher failed to work properly resulting in a much larger burn than if the extinguisher had worked.
Fire extinguisher failed to discharge. As a result, the fire grew in size.
During annual fire extinguisher training we are required to put out a controlled fire. I pulled pin and gave a short burst of extinguisher, I then proceeded to approach base of fire and attempted to discharge the remainder of bottle but nothing happened. Squeezing the handle did nothing, it appears the valve is stuck. I had fire fighter personal try also and nothing.
A Kidde fire extinguisher blew its top. It was an ABC type. The model is Fire Away 460 and is not listed on the Kidde recall list. The pressure has always been in the green region. It was stored in the basement where it was not subject to heat. It was not being used at the time and I'm not aware of it ever having been dropped. No injuries, just a $20,000 cleanup bill.
My mom got me a fire blanket from Prepared Hero. The blanket is a woven fiberglass matt. It was in the back of my work truck, I never opened it or anything like that and it was shedding fibers of glass that embedded themselves deep into my hands. This started happening right after Christmas, and it took me 3 months to figure out where it was coming from because the glass fibers are basically invisible and I never suspected the fire blanket to be the culprit. The company has no warning label of any kind on the product, and they make it seem like its perfectly safe to have it out of its bag. Theres a picture of a person who uses theirs as a floor mat under the grill to protect the deck from embers. This is an insane thing to do. The glass fibers drove me to near insanity as I had no idea where they were coming from and long fibers of several inches buried themselves in my hands by the thousands, or so it feels. And they dont come out of the skin. Ive been to several doctors because of this. My hand got infected with MRSA. Its now august 13, so its almost been 8 months, and my hand still bothers me every day. The reason this is so frustrating is the fact that the company not only fails to mention the danger which they are well aware of based on some digging I did, but they actually actively hide it from customers. Had I known the blanket was the source I would have fixed the problem immediadtely rather than getting hammered by it day in and day out for 3 months. My hand is in a crazy state right now. I will likely never be able to use it properly again. And the glass causes chronic inflammation which can lead to cancer, so I do worry that this company is going kill me. I think they need to be forced to put a warning on their product that says, this product is made of glass fibers which shed off of it, are invisible, and can penetrate flesh deeper than skin layers. The glass is coated in silicone which is notoriously resistant to adhesion or dissolving. It will
