I used the outdoor swing by a company called The Salty nest. My toddler weighs 32 pounds, the swings are supposed to hold up to 90 pounds. The rope on the swing ripped, causing my toddler to fall face first out of the swing. My toddler hit her head on the ground when falling.
Children's rope swing with single braided rope. When spun, the braids easily loosen. Fingers/ hands can easily be injured when caught in the braids.
In summer 2017, I purchased the Little Tikes Tree House Swing Set and installed it according to the manufacturer's instructions. Unbeknownst to me, the instructions contained an engineering error that resulted in very serious instability of the entire swingset and danger to the children uses. Over time, it started shaking and vibrating and became impossible to swing on, because the railing and posts were mostly disconnected an dready to fall apart Four bolts holding the railing to the posts became loose and unstable because they were not installed with washers on each side. The swingset was installed according to the manufacturer's instructions, which called only for a plastic "bolt cover." Four of these are used, and all four broke. I only realized the company's error when I was looking for replacement parts and saw the instructions for the "Endless Adventures Swing Set," which has almost identical layout and engineering. This nearly identical product is shown with washers installed on both sides of the swingset. I have communicated with the customer service for Little Tikes and they have not yet taken responsibility.
Child was swinging in baby/toddler Little Tykes swing and was buckeled in. The swing back where the buckels attach suddenly cracked and a large piece popped out leaving child to fly forward and not be buckeled in anymore. Adult caught child before he could fall on the ground.
At the Waldron elementary school in Waldron Indiana [REDACTED], age [REDACTED] decided to utilize a play booster for his enjoyment. He jump for the ring from the takeoff platform. Priior to the day of [REDACTED] fall, he had used the play booster. However, on the day of the fall, [REDACTED] first use of the play booster resulted in him falling. [REDACTED] jumped from the platform, he grabbed the wheel and as his feet swung forward [REDACTED] lost his grip on the wheel. This resulted in him falling. As he fell his body was essentially parallel to the ground. It is likely his body did not fully straighten resulting in a perfect horizontal plane, but it likely twisted. This explains why the right side of his head hit the platform.
My grandson and his cousin were playing on this Flexible Flyer Play Park Swing Set, Model 46604T, when one of the four bar attachments to the lawn swing came apart and they crashed to the ground. My wife and I bought this for them around October 2016 for their Christmas present. The problem is that the phillips screw goes into a threaded sleeve that has a hex head on it with a lock washer. There are two pieces of black plastic that straddle each of the four bars and this screw and sleeve hold it all together. If you tighten this up too much, then the swing will not swing smoothly because the black plastic is being squeezed too much and making a terrible noise. They should have used a longer screw that would have bottomed out or tightened in the sleeve. We bought the same model for my son's kids and he just had the same thing happen with his 5 yr old on the two seater air glider. Only difference was that his son hit the ground hard and ended up in the ER this week. Xrays showed that he had no injuries and was released to go home. My son will be contacting also.
We have the Rainbow Play set, purchased Summer 2009 with the yellow plastic trapeze rings. The rings broke in half while my son was swinging on the - he is 60 pounds. He flew off and landed on his back and hit his head on our ground. It appears as if the rings broke in half at a seam in the plastic. This should not happen.
The rope on the bottom left part of the swing came unglued from the piece of plastic holding it in, leaving baby dangling.
