Was beginning to grind piece of metal with a handheld drill master die grinder. The blade chipped & the grinder flew out of my hand into my ankle. It cut through tendons. This tool does not power off when it comes out of your hand. I had to reach over to turn the switch off. I had surgery Sunday evening to repair tendon after pieces of the blade were removed from my ankle. Handheld tools should all have a safety feature and should power off immediately if not being held.

The submitter is reporting the unit for posing an injury hazard. She said during the first use the unit fell apart in three pieces. She indicated that it occurred while sharpening an object at 3300 RPM. She sustained a laceration on her left thumb but she believes the injury could have been worse if she had not been wearing protective gear. At the ER they stitched her wound and took x-rays. Her son sent a letter to the firm reporting the incident. She said the firm has not yet responded.

While using the Metabo grinder the switch was released to stop the blade but the tool kept running. In reaction to this the operator somewhat lost control and the blade struck a steel member causing the blade that was still under full power to come into close proximity of the operators body in what is being considered a near miss in bodily harm by a hand power tool. No injury occurred.

On 8/30/15 I was using this grinder with wire brush attachment cleaning out a tank for my boat. The auxiliary handle broke, the grinder kicked up catching me in the face and chest. The machine didn't shut off and I had to pull the plug out of the socket to stop the machine. The handle that broke is very thin construction and may have been flawed. The on-off switch does not automatically shut off when released (as OSHA standards may require). The on-off switch is located on the motor. The motor gets hot making it hard to hold on to this tool securely. Luckily I had the wire brush on this tool and not a grinder or cutting blade. Otherwise, instead of just bruising and scrapes the blades would have cut through my cheek and chest and I might not have been able to cut it off by pulling the plug & I would have been dead. Note: Item was being used IAW owner's manual and safety instructions. See Attachments: A, B and C.

Bought a Porter Cable PXRA2676KIT Multi Surface Restoration Tool Kit. Upon using it it started to make grinding noises. Then sparks flew out where the sanding drum is then hot metal parts flew out. Luckily I was wearing safety glasses. I was able to save one of the pieces that flew out. The other one is lost in the yard. I am trying to contact Porter Cable buy so far have only been put on hold. The part appears to be part of the main housing, is metal and was ripped out of the plastic housing by the action of the sander. The parts that came out are a piece of metal rod approximately 1/4 in diameter by 3/8 inch long.

I was using an angle grinder with a brand new Harbor Freight cut off wheel. Half way through the metal pipe the wheel came apart in about 6 pieces with one of them flying back at my hand cutting my right index finger at the knuckle that required 8 stiches

4.5" Angle Grinder from Harbor Freight the weld on the metal guard keeps breaking. This the 3 or 4th one I have bought and the guard breaks while in use. Also, there is something wrong with the internal safety mechanism. The grinder will shut off during usage and if you don't remember to turn it off it will intermittingly turn on while hanging on tool hook or holding.

At an Ironworkers Shop here a trained ironworker secured the WESTWARD twist wheel 4” stainless steel onto a [REDACTED] grinding tool. The max rpm of the wheel is listed as 20000 and the grinding tool lists the max rpm of 12000. Upon the beginning of the grinding operation the wires from the WESTWARD began to become released from their source. The ironworker fortunately was wearing all of the appropriate PPE and was not injured in any way. The worker removed the first wheel, inserted a second wheel from the order of 48 and the same result occurred. A significant portion, 43 to be exact were returned to the local supplier (Grainger) before I was made aware of the situation so I only have three currently in my possession. I spoke to the Albuquerque Grainger Rep, then Grainger International, who then could only offer me to file a “customer concern” and that my reporting of this disengaging to the wires was an “isolated incident”. They said that they felt my call warranted only a “customer concern” as it was an isolated incident even though the wirers came off of two out of the 48 we ordered.
