Working on a Werner model ALMP-20IAA ladder configured in the step-ladder mode when the rope side locking mechanism broke and the ladder fell. I suffered displaced fractures of my right arm requiring surgery for internal fixation of the ulna and radius head replacement as well as contusions and soft tissue injuries to my head, right hip and leg, and left hand, and a broken nose. Injuries left me with chronic pain, limited range of motion in my right arm and left fingers, numbness in my right hand and forearm and sinus issues.

I own a Keller Modle 910 10 foot stepladder. It is a Yellow Line Industrial Type I unit with a part number of 421Y-10. The ladder is rated at 250lbs. I weigh 180lbs. I was working on the ladder in my yard when, without any warning, the second from the top rung let go. I was lucky and landed on the next rung down in a sitting position.
I was using the step ladder to reach items that were on the top of my kitchen cupboards. I keep the ladder in the closet on my patio, and am familiar with using it. I opened it as I normally did, and heard the click that told me it the latch was engaged. I retrieved my item and stepped down to the second step and the step collapsed. The entire ladder tried to fold back up. I fell from the second step and landed on my bottom, using my left hand to brace me. There was a plastic trash container behind me that I fell into, which left a large bruise on the back of my arm, as well as tingling and numbness on the left side of my body (I fell more to the left side of my body - I was holding a large glass bowl in my right hand. The bowl didn't survive the fall.). Although I did not seek medical attention when it happened, I do plan on seeing my primary care physician because of numbness and tingling in both arms, especially on the left side.
I purchased a Little Giant Mega Max ladder ( product date code: 10272S) from Costco in [REDACTED] in April of 2017. Last week on 8/3/2017 at my home in [REDACTED] I experienced a failure in the ladder's locking mechanism which resulted in a severe bodily injury. I had deployed the ladder in the extension ladder configuration when the locking mechanism disengaged and the ladder slide downward and twisted in one motion. In that sudden movement my right ankle was caught between the twisted rungs of the ladder and I simultaneously hit the ground awkwardly with the rung of the ladder striking my shin. The resulting injury was a fracture of my ankle, that will now require a surgical repair. I believe a further Investigation should be undertaken to reduce risk for other consumers. To whom it may concern: On July 13 2012 I was painting on your ladder at the [REDACTED], in Lake Ariel Pa. 18436. This ladder was a new ladder bought specifically for painting. I was on the second to the top step when the ladder twisted somehow and went out from underneath me. I fell backwards about 8 to 9 feet in the air and landed on the concrete floor. I injured the whole right side of my body. I knocked the wind out of myself and couldn't breathe for a while. When I got my wind back the pain was unbelievable. My right arm swelled up like a balloon, there was pain in my ribs and lower back. The ambulance took me to the hospital because they thought I had a broken arm. The x rays showed I didn't have any broken bones which was a miracle. What I did have was a lot of pain. I couldn't bend my arm for two or three days to eat, that's how bad this was. I still can't use my arm fully, and to this day I still have a lump on it. I believe this ladder is a bad design and should be fixed before somebody gets hurt really bad I was lucky, if you want to call it that that I didn't break my back or get killed falling that far on to a concrete floor. Your ladder has metal braces on every step except under the bottom step which is the most important to stabilize the ladder and it has a plastic brace. I believe this is what caused the ladder to twist. This ladder, like I said, was on a concrete floor which was perfectly level. I am enclosing pictures of your ladder showing how it cracked at the bottom and along the seam. This is a fiber glass ladder and should have never done this. The bottom aluminum step is also split.
Yesterday morning (15 Aug 2017) I was standing on our Werner 6 foot Type II Medium Duty/Commercial use non-conductive Fiberglass stepladder. When suddenly it collapsed under me in a split second throwing me off to the right. Both cross braces had bowed with vertical cracks in the fiberglass appearing where the cross braces are attached to the ladder. The skin on the back portion of my right elbow was shaved off apparently by the bowed out cross brace on the right side of the steps as I fell to the ground. I also hit my head on a lawn border brick. I was taken to the local emergency room by ambulance where my complex lacerations were sewed up. I have always been safety conscience and ensured the ladder was fully deployed and on level ground. I weighed 231 pounds yesterday which is 6 pounds over the specified weight limit of 225. I believe I was standing on the third step from the ground and in the process of moving to the fourth step.I do have pictures of the ladder following its collapse, but am having trouble uploading them to your site due to the special Flash Player requirement. This morning I spoke with an individual in the construction business regarding my accident yesterday. He informed me the construction company he was with threw out this stepladder as soon as it reached five years old. Apparently, they were aware that the fiberglass in this ladder tends to lose its strength and fatigue over time resulting in this type of accident. My ladder was probably about eighteen to nineteen years old. The individual looked at my damaged ladder and said the same thing had happened to him where the cross-braces suddenly buckle outward, and their attachment points in the fiberglass exhibit vertically running cracks.
