I purchased a toddler playground set from Target online in November 2024. When my family set it up outdoors in March 2025, we discovered that the netting on the basketball hoop and soccer goal was disintegrating. The material breaks down into fine dust when touched or rubbed, releasing particles that can become airborne. The product is marketed as suitable for children, and the manufacturers Q&A states it can be used outdoors, but nowhere does it warn that the netting may deteriorate into dust. This creates a potential inhalation hazard, especially for young children with developing lungs. The playground was being used by a toddler for normal play activities (basketball/soccer goal). Although no injury has occurred yet, the risk of children inhaling synthetic fibers and dust from the netting is serious and could cause respiratory irritation or other health impacts.
To whom it may concern, I am a former education assistant of 7 years for [REDACTED] in the [REDACTED] area of [REDACTED]. I am writing to address a repeat-injury inducing piece of playground equipment known as Gaga-Ball https://www.gagaballpits.com/gaga-ball-pits/ During my 2nd year at [REDACTED] in 2017, school administration decided to have one of these wooden pits installed atop of rough-edged asphalt concrete. The objective of this game is to strike the ball below knee level, with the goal to hit the ball at other players legs to eliminate them from the game. The problem with this however, is that the students would strike their hands on the asphalt, creating disastrous hand injuries. These injuries were often very bloody and messy, requiring the schools education assistants and office staff to tend to the injured player/student. I have informed [REDACTED] principal directly about the numerous incidents, letting her know that the game presents a major liability, and that the Ga-Ga Ball asphalt surface must be reinforced with a protective surface, such as rubber or [REDACTED] to reduce injury. The principal of [REDACTED], has chosen to do nothing. Their response to me was that installing rubber, or any other protective surface would be too expensive. If the protection of students is too expensive, then it seems logical that maybe the wooden octagon pit should be removed altogether. As school employees, we undergo bloodborne pathogen training modules at the start of every single school year. The conditions of the game, Gaga-Ball are unsuitable within the [REDACTED] district, as the game repeatedly exposes school employees to bloodborne pathogens through repeat ripped flesh injury. This incident(s) is among the primary reasons I decided to leave [REDACTED] last March. I recommend [REDACTED]s GaGa-Ball pits be investigated for safety hazard. It was mentioned in [REDACTED] group that someone had purchased a CedarWorks indoor playset on [REDACTED]. She was asking how to secure the blue 'balls' which are found on top of the indoor play sets. It was told to her in the comments that they are a strangulation hazard which is why they are no longer allowed on cribs. A necklace, hood, shirt, cape etc could get caught and the child could suffocate. The units are currently for sale on their website.
We bought a foldable ball pit thinking it was safe for children. My 1 year old daughter was playing around the ball pit, fell and landed on the top edge of the ball pit. Her whole cheek was red and was left with a horizontal bruise on her cheek
On May 17, 2025, a six-year-old girl was playing at the McDonalds at 32 Broadway in Norwood, MA, in what appears to have been a McDonalds Play-Place. The child fell off of a rolling cylinder which was part of what appears to be a kid conveyor, manufactured by Soft Play, onto her shoulder. The rolling cylinder, shaped like the Soft Play log roll and composed of a synthetic material, was turquoise in color, and was horizontally suspended off the ground. The surface the child fell onto had no padding. She was in extreme pain and later learned that she had fractured her clavicle. She was treated at [REDACTED]. The play equipment that injured the child was a turquoise, synthetic, mounted roller shaped like a cylinder or barrel suspended off the ground, believed to be a kid conveyor in the McDonald's Play-Place, and believed to have been manufactured for McDonald's by Soft Play. A six-year-old would not be able to traverse over this piece of Play-Place equipment without going on top of it and climbing over it. Because the equipment would spin, a child climbing over could be spun off the equipment and fall off. The surface below this piece of play equipment had no protective padding so there was nothing to mitigate the bodily damage sustained if a child fell. The equipment is blatantly dangerous and cannot be safely used by children, and there are no padding or soft landing precautions in place for when a child inevitably falls from the equipment. McDonald's did not post any signage to warn parents of the dangers of this piece of play equipment and did not supervise those using the product. The actions of McDonald's and the design of Soft Play resulted in the serious injury of a six-year-old child. McDonalds was informed of this serious injury the same day it occurred. I purchased 11 bags of Quickcrete Play Sand to make a sand box for some children that are staying at my house for a week.I bought Play Sand because I have had a good experience with it in the past in similar situations. My concern is that there were quite a few angular, sharp pieces of some white rock uniformly spread throughout the ten bags I used. They are fairly large (20 to 40mm) and seem like they could be a seriously problem with toddlers. I still have one bag left that I will keep closed and have kept the pieces I was able to strain out of the sand.
a nine-year old boy was playing on the "Spring Across Bridge," manufactured by Landscape Structures Incorporated, when he fell. The Spring Across Bridge has thick metal coils that are connected to several relatively narrow metal beams, causing the Bridge to toggle back and forth in different areas when children walk or run along it. There is no housing unit or covering in place where the round coil meets the square beam, leaving the four metal corners of each beam exposed. This appears to be the intentional design of Landscape Structures, rather than an alteration by a user or consumer, although this is not certain. When the nine year old boy fell, his shin hit the exposed upper corners of the metal beam, causing a large V-shaped laceration. one of the arms of the "V" was approximately 4 centimeters long and the other arm was about 3 centimeters long. The laceration was so deep, a chunk of flesh and muscle tissue was torn off.
My 5 year old son was playing on the playground, walking on a playground piece that connects two parts of the playground. There was a metal bar across the top of the walkway where it connects to the playground that matches the color of the playground and is thin. My son did not see the bar and went face first into it, knocking his front teeth loose. I believe this bar should at minimum be a contrasting color and more ideally a plastic surround around the bar. It was also below his line of sight when walking.
