We had our fire dampers checked in March, out of 221, 128 failed to close and latch as they should. I contacted Greenheak Fan worked with a [REDACTED], and eneded up getting all replaced at no charge. I had sent them 5 dampers, to check out, and they said they had cut the material too short in the construction process. I also contacted [REDACTED] with the UL, and he also helped in getting the replacements. My concern is how many other Hospitals, or nursing homes out there have these bad fire dampers in them, that will not work per design when there is a fire.

Keen Home Smart Vents - these are ceiling mounted HVAC vents that open and close via temp sensors through a vent to close off/on HVAC throughout a room. They weigh 2 to 4 pounds and have (4) AA batteries of weight too. These vents are manufactured in two pieces with one piece being the actual vent portion mated with a housing type bracket that is installed into a trim kit and then mounted on a ceiling or wall. The housing and vent are mated together with just (6) strips of double sided tape. So over time, 12 to 18 months the tape fails to hold the vent in the housing - and the entire assembly just falls crashing down on top of whatever is under it. If it's a pet, elderly person or small child they could be easily injured. There is also a deco vent cover held on by 4 magnents - these magnets are glued and the glue fails too - on cases where the vent hasn't totally fallen out, the vent deco cover ha when the glued magnets fail. Two of four of these vents failed in my home and crashed to the floor. I notified Keen Home twice and they refused to respond. Also, their eCommerce site, doesn't have a place to open help requests and there isn't a phone number. http://help.keenhome.io/en/ The company doesn't want to be bothered with or cares if someone using their product is hurt. I feel this product is a safety hazard. I had two incidents, no injuries. One was April 12th 2019 and July 1 2019

Had new Roof put on with power vent to prevent mold and in less than two years there is substantial mold which needed to be remeated. This was the most expensive roof with new technologies for better roofing. The roof was $15000 on a 1300sq ft ranch house with hip roof. 50 yr warranty but not standing by product.

I recently purchased a Deflect-o Hardware Dryer Vent Kit from Home Depot. When I tried to install it, the plastic zip ties included with the kit do not work properly. The zip ties are used to attached the duct to the dryer and vent pipe. The zip ties included with this kit do not hold tight or have the ability to come loose. This poses a huge health and safety risk! This product should be recalled for this reason!

Honeywell MS4120F 1006 actuators were being used in the Lincoln Tower fire suppression system in Bellevue, WA. During an annual test for the fire department, a majority of the actuators that were supposed to fail open to allow relief air out of the building, failed to operate properly. Out of 9 actuators tested, 6 exhibited the same failure. The failure was occurring at the main gear shaft. This metal shaft was intended to be mechanically pressed into the spring case and become displaced and loose. The shaft had been only partially pressed into the spring case with allowed the shaft to 'rock' in the improper joint which eventually lead to the shaft coming free. Once the shaft joint was compromised, the interior cover bore the eccentric loading from the gear shaft which it was not designed to withstand. The gear cover then broke loose as well, causing the gears to move out of place, grind on each other and eventually causing the gears to become stuck and unable to move.
