The product in question is an indoor/outdoor ethanol fireplace. This incident involved two people, the victim and her male friend. The victim was sitting at the end of a dining room table. Located in the middle of the table was a Moda flame Vigo indoor/outdoor ventless fireplace (see picture). The friend stated that they observed the flame going out so he grabbed the bottle of fuel (see picture) and poured more ethanol in the fireplace. When he did this he said the fireplace flashed on fire and in turn, there was also a fire on the table. He grabbed a towel and put the fire out, but he mostly described the fire as being a flash fire. Shortly after the fire was out on the table and fireplace he said the victim told him that she was on fire. He reported the front of her dress was on fire and the flames rapidly traveled up to her face. She stood up and tried to pat it out, he ran over and helped. She went up to the shower to cool the burns and he called 911. The victim sustained 40% partial thickness burns mostly to her torso. She passed away at the hospital several weeks later due to treatment complications.
Built Home through construction firm Lombardo Homes Saint Louis, MO. They used subcontractor CB Bennett. They installed a faulty fireplace insert in our home which we closed on 11/7/2012. A monessen hearth fireplace insert, lining and flue cover, mane, granite and insert for log. It has never worked and have had several unsuccessful service calls.
Dear Mr. [REDACTED], I came across your name on the CPSC website and assume you might have knowledge that could be helpful to my clients, both of who suffered serious, third degree burns in connection with these product (See attached photos). I am aware that the CPC is or did consider the safety of these kinds of products. I cannot tell from the website the status of that effort. I generally am interested in identifying the best persons to perform a technical evaluation of the products involved in my clients injuries and wonder if you might be able to direct me to someone specifically knowledgeable in this area. I had a Skytech model SKY-1001, date code 9601 that burned and started to melt. It happened one evening before we went to bed. It's a good thing that we weren't sleeping; If we had been asleep, a fire may have started. There was a burning smell that I thought might have been either a computer or a TV, but it was the SKY-1001 receiver that was mounted in the wall. It was so hot the batteries were sizzling/boiling. I pulled it out of the wall with some insulated gloves and set it outside and removed the batteries. I sent the above report to the Support/Quality team at Skytech. I asked them if they wanted me to return the burned/melted unit so they could perform failure analysis, but they did not want it back. I'm writing you in case you would like the Skytech unit and to see if there have been other similar incidents reported. If we had been sleeping, and the burning/melting continued, a devastating fire could have resulted.
The consumer indicates that he turned his fireplace on in the morning however thirty minutes later there was a loud explosion. He was sitting in a chair adjacent to the fireplace and his dog was laying on the floor across from the fireplace, the fireplace glass exploded and shards of glass had flown all over his living room. The consumer attempted to contact the Manufacturer but found out that they are no longer in business and were taken over by Inovatiu Heaph Products. When the consumer contacted the new company and explained the situation, the representative he spoke to informed him they could not assist him as the unit was no longer warrantied. The consumer states that the unit was used for two to three hours daily during Winter. The consumer plans on keeping his fireplace for at least the next thirty days in case an investigation ensues.
There was an explosion, while the consumer was away from his home. There was an explosion inside the burning chamber of the fireplace and it blew through the glass front. The front glass was broken. This is his first incident ever with the fireplace. The fireplace had been used earlier in the day (length of time unknown). However, the thermostat had just increased past the ambient temperature. There were some fragments of glass that spread across the home, but there was no damage outside of the fireplace. Someone in the vicinity loss temporary hearing after the incident, but it has cleared up. This person may have been sitting within 5' - 10' of the fireplace at the time of the incident.
This Lennox Montebello see through fireplace was installed in 2020. We've used this fireplace for 4 years without any issues. Yesterday, we ran the fireplace for about an hour on HIGH. After about about an hour, we heard crackling in the wall, and smelled "burning" wood smell. Called the fire department and opened up the sheetrock below the fireplace to see a fire coming out of the bottom of the fireplace box. It was burning from the bottom of the fireplace. It had ignited everything under the fireplace. The fire department checked the gas line and said it was fine, and not the cause of the fire. I called the manufacturer, Innovative Hearth Products, which claims that sometimes when the ducting above the fireplace pushes down on the top of the firebox, it will cause the glass to leak air, but my ducting is not pushing down, it is supported and installed correctly.
We recently purchased the TerraFlame Lloyd Modern Gel Fuel Fireplace which states it is clearly meant for indoor use. This product uses three gel fuel cups to create a indoor fireplace. Immediately we found the fireplace products a terrible chemical order when burning the gel, and people in the room started complaining of headaches. We tried a second time and it produced the same result. There no no specific injuries, but I could see if we kept this burning longer (the gel fuel is supposed to last 3 hours) it could have made people sick.
