I was up.on the 20ft Aluminum D1120-2 extention ladder max weight allowed was 200lbs ( I was 125lbs) I had the ladder extended 17 feet up and was standing on the 6th rung from the top fly section all the sudden I heard a cling noise so I decided to get back off the ladder so I stepped down the next rung (7th) and the ladder started pulling away from the wall and was bending towards me. The ladder then tore apart and tore open the drywall on the home I was working at I then was flipped around with my foot between the rungs and the ladder we t down causing me to fall and broke my left talus bone in my foot my foot was turned the wrong way and arched like up on my tip toes, I ladder on the ladder with it broken on top half and my back against the wall. I hurt my back badly and cut my elbow it was very scary and I could have died. After years of surgeries I now have had to have part of my leg amputated in 2014. My career as a house painter is over and live off disability which isn't enough to support myself.
keller industries all aluminum ladder. during regular use one of the lower treads collapsed apparently due to corroded rivets. No major injury resulted from this incident however this issue with the thread rivets may likely cause more serious injuries. Have photos in support of this incident.
Monday, September 22nd, 2014 the caller was on his 24 ft ladder purchased in June 2006 picking apples when the ladder broke 5 ft from the top sending him to the dirt ground. He lost consciousness briefly (possibly 15 minutes) when he awoke his wife advised him to contact his son in law who took him to the VA Hospital in Johnson City TN emergency room. He was not admitted but was examined, ex-rayed, diagnosed with broken left ankle, badly sprained right ankle. His knees are scraped up but the right knee and elbow are the more painful. A few small bruises are on various parts of his body. He contacted the manufacturer on Tuesday, September 23rd, 2014 and received a return call this morning September 25th, 2014. He did not obtain the name of the representative that returned his call but she advised him to send pictures of the ladder to her. He will also send pictures when he returns this report.
Attorney representing client stated that the Little Giant Leveler Type 1A Ladder (adjustable ladder) made exclusively for [REDACTED] failed unexpectedly similar to the recalled ladder failures involved in CPSC Recall #17-113 dated 03/23/2017. The ladder's rapid lock fasteners failed after the client adjusted them when suddenly the ladder collapsed while the client was on the ladder in August 2021 and was preparing to adjust the video camera's in his wife's store. The client sustained a fracture to his left wrist and required surgery that included pins and screws to repair his injury. The client also sustained contusions to his leg. The attorney hired an expert who tested the ladder and revealed that all of the rapid lock fasteners failed on the entire incident unit. The attorney has detailed photographs and a video of the incident. The client has limited use of his left hand and wrist.
When using the extension ladder the structural integrity of the last side failed causing me to fall approximately 10 feet. I went to have x-rays performed but they found no fractures and no brakes but since have experienced severe low back pain as well as neck pain since the fall since learning of the recall I'm now returning my ladder go home Depot . Something should have been done to let the public know this danger without having the research Werner ladder recalls on my own.
Hello, I just wanted to write you real quick. I recently purchased a Werner-MTIAA-Aluminum-22-ft-Reach-Type-1AA-375-lbs-Capacity-Telescoping-Multi-Position-Ladder from Lowe's Home Improvement store. I paid $178 and was very excited to have a versatile utility ladder that could meet all my personal needs. I purchased it on July 25th, 2018; not two months yet. So far I've only used it for two things: getting into my attic through the man hole in our garage ceiling and for installing some directional antennas on the side of our house for our 4G wireless internet. Today, I was very disappointed, a little scared, and injured on the ladder from Werner and can't believe something like this would happen on a practically brand new ladder... I had to remove the antennas I had previously installed because we decided to go a different route with our internet services. So I brought the ole ladder around the house, opened it out all the way straight and ensured the clips locked in place, and then I extended the telescoping on each side equally to achieve the desired height I needed. I am always a little nervous on ladders because I've heard and read lots of horror stories, my brother in law actually had a close friend who passed away from falling off a ladder. So I made sure that everything was used properly on your ladder, everything was locked in place, and the C clamps from the telescoping part were secured inside the holes for my desired height. I got almost to the top of the ladder as I began to feel something give and then completely release. The aluminum metal broke and bended at one of the ends where the telescoping part comes out. It's kinda hard to explain through a message so I took pictures. I was lucky that I was able to fall over to the right side where there was nothing in the way like the wooden fence on my left. I fell about 10-12ft from the ladder but I only came out with missing a small chunk of flesh from my right thumb as it caught on the metal whil
Consumer stated that while using the ladder extended to 16ft the locks didnot hold causing the ladder to collapsed which caused her to fall to the floor, she received injuries to her back and to the right of her ribs, sprained left foot, torn ligaments to her right foot and bruises to both elbows. She was not taken to the hospital or received any medical attention. Consumer stated that she will be making an appointment to visit a physician. 9/6/11 Consumer stated that she contacted the manufacturer and was advised that someone would contact her, but no one had as yet. Consumer is concerned that this ladder is a safety hazard and should be reported.
