Submitter says that the can exploded under the sink. Foam from the product got on everything else that was being stored under the sink. Caller attempted to remove the product from under the sink and the foam got all over his hands. Missed two days of work due to not being able to remove the foam. Submitter contacted the firm and they advised that once the foam drys it is unable to be removed.
Consumer stated after he had used the spackle to patch holes in preparation for painting the room, approximately 1 hour later he and his spouse smelled a chemical odor which he believed caused them to develop severe headaches, difficulty breathing and chest pains. They were not able to spend the night at the house. 6/25/11 Consumer took his spouse to the Emergency room at [REDACTED] in [REDACTED] where she was given oxygen and some kind of treatment (not sure what kind) by the physician and released. There was no diagnosis. Consumer stated on 7-12-2011 his spouse took him to the emergency room at the same hospital for treatment, where he was treated and released. Consumer stated that because of the fumes that this product released, he and his spouse were not able to live in their home since 6/24/11. He has had surface and air samples for the interior of the house tested by a company and he was advised that there was formaldehyde 20, sodium silicote 4000 ppm and sodium borate(not sure of the levels). Consumer contacted the manufacturer on 7-8-11 and 8-3-11 and was advised that they did not believe that he and spouse illness was caused from the product. Consumer feels that the Spackle is a health hazard and should be investigated. 10-11-2011 Consumer added: The chemical(s) in the spackling was unknowingly transferred to my daughter's apartment. The chemical(s) are in my clothes and truck as well as my wife's clothes and car. My granson has become sick and my daughter became sick and had to go to the hospital emergency room.
The product "Flex Seal" (as seen on TV) was sprayed and soon after I came down with a severe lung infection. I believe [REDACTED] are used to make this [REDACTED] product they call "Flex Seal". Please investigate.
74 yr old caller is reporting that she is unable to read directions/warnings on label of sealing product purchased for concrete/brick. She used this product inside her office to seal a wall where mortar is coming down. Caller wears glasses, states that she can usually read most containers warnings or directions but could not read them on this one gallon plastic container because the type is so small. She believes that she may be alright but would like to report while requesting that labeling be required to be larger. She has not been seen by any medical professional, does not feel ill. She spoke with the manufacturer today, July 23rd, 2011, did not obtain the name of the representative she was speaking with the representative said she would note and tell her people. Caller has Asthma and hopes that she didn't do any harm to herself.

Introduction and Summary of Safety Concern This complaint concerns the improper indoor application and misleading marketing of OSI QUAD Advanced, a solvent-based construction sealant manufactured by Henkel and distributed through major retailers including Lowe's, [REDACTED]. The product was applied inside my home during a Lowe's window installation project, despite being intended solely for exterior use. The chemical exposure required subsequent product removal. Manufacturer documentation and misleading retail advertisements present a serious risk to consumer safety, especially when installations occur indoors. 2. Background and Incident Summary In 2022, I hired Lowe's to perform a full window replacement at my residence. The installation was subcontracted to a third party, and solvent-based OSI QUAD Advanced was used to fill large interior gaps around 24 windows without my knowledge or consent. I later learned that Lowe's subcontractor had reportedly used this sealant indoors on every window installation he performed for the company. This was confirmed to me in a phone call with Lowe's Installation Support. Shortly after the installation, I contacted the manufacturer, Henkel, regarding the strong odor in my home. Henkel later confirmed in writing that OSI QUAD Advanced is not approved for interior use and must be fully removed if applied indoors, as covering it with other sealants will not mitigate the chemical exposure. Lowe's initially denied responsibility but eventually removed most of the product after inspection. Nearly two years later, I discovered that multiple retailers had advertised the product as suitable for both "interior and exterior windows"-a claim directly contradicted by the manufacturer's safety guidance. Lowe's later admitted this in writing and quietly revised its website. 3. SUMMARY Over eleven months ago, I followed instructions on the product container to wear eye ware and gloves, when using the Great Stuff product. Unfortunately, this product dropped from my wrist up to my shoulder. In reading the information indicated on the product container in a separate area on the can titled WARNING/DANGER, I later learned that the product is "sticky" and can only remove it "mechanical means, once it dries on skin. It also indicated that soap and water be used to wash it off. This does not work. It did not wash or scrub off. The only way I can remove is to scrape it, thereby also removing a layer of my skin as well. After damaging my bedding from the bleeding, I went to a dermatologist (at my expense) and learned that this was drying out my skin and that I needed to use a skin moistener Instead. It is over a year now and not completely removed from my skin. I discovered that the best time to scrape some of it off is when I get up in the morning because; blood tends to flow away from the extremities at night. It definitely does not wear off! Given the difficulty of removing this product from skin, I believe that the container instructions are inadequate, deceptive and remiss in properly warning the user about this potential situation. Using "sticky" to describe this is woefully inadequate. I notified the manufacture in writing however; they claim that they are in full compliance with Federal regulations. I would appreciate hearing from you, if you agree that the container is in full compliance, as the manufacture claims. P.S.
