(02/2012) The consumer stated that he and his wife experienced dizziness, nausea, and headaches. They went to the emergency room at St. Luke's Hospital in Chesterfield, MO. They were told that the situation was a result of carbon monoxide. The consumer stated that he spent the night in the hosptial for observation because he has a heart condition. His wife was released after two hours. (02/2012) The local gas company came in to test the product. They found that there was 32 parts per million of carbon monoxide coming out of the fireplace. The gas company disconnected the gas valve to the fireplace. 02/2012 The consumer contacted manufacturer and told the product was ok. UPDATE: The ODS on the fireplace did not shut off the fireplace when it started emitting carbon monoxide.

Refer to the attached photos of the rating plate for a gas logset we have in our showroom. Note there is no hole punched in the metal tag for permanent mounting to the burner as required by ANSI Z21.60a. See attached photo from the ANSI std. It must be attached- not thrown into the box. Also, there is no tag with the lighting instructions for the end user. I previously reported another Grand Canyon logset in our showroom where the tag was labeled LP but the orifices and regulator are for NG. We refused to install it while the wholesaler and mfr. are refusing to credit it back. We are done dealing with Grand Canyon as they obviously have no quality control systems in place. However, as the listing agency, YOU should be periodically inspecting their facility for quality assurance of the listing. I find OTL has failed twice in that regard. I will be reporting both issues to the CPSC for their consideration. One recent see-through LP burner had a gas leak out of the box in excess of the Lower Explosive Limit. This unit did not come with a rating plate so no serial #, data, etc. but the box(since discarded) was labeled listed to ANSIZ21.60
I have a Heat & Glo gas fireplace place that is controlled by a remote. Several times the fireplace has been found running while the controls were set to off. The manufacturer did have me reset the code on the controls and were helpful the first time I called. When this did not work I contacted the manufacture again. The second customer service rep was flip and informed me the fireplace could run for 30 days straight and not be a safety issue. The local company whom I purchased the unit from, Birch Hearth, informed me the controls were set to thermo. After several discussions, I produced photo evidence that the controls were not in the thermo position. Then and only then would they replace the remote control and sensor. They have done this and I am waiting to see if there will be an issue. No one I have dealt with seems to think this is a safety issue. To me a gas appliance that randomly turns itself on while the controls are clearly in the off position, could be a safety issue.
In 2012 we requested service on our HeatNGlo GEM 36 gas fireplace. The technician found a single crack in the ceramic surround enclosure. We asked at that point to have it replaced. We were assured the crack was normal and it was no problem to continue using the fireplace. In 2015 my wife began having headaches when using the fireplace. We shined a flashlight in to the fireplace and found the crack has "spider webbed" everywhere on the top, sides and back of the ceramic enclosure. We immediately visited the local HeatNGlo distributor and requested a visit. They came the next day took some pictures and stated the fireplace probably needed to be condemned, but that they needed to confirm this with the factory. The factory emailed and stated there was a patch kit to solve the problem. We took more pictures, forwarded these to the local distributor (Groth Gates Inc.) and the HeatNGlo rep. along with several questions concerning the patch kit and application. We then received an email stating the damage was beyond repair and not covered by warranty. This unit has become very dangerous and unsafe. This began within the warranty period, but we were lied to about this problem. We have since found out from another distributor in Salem Oregon that they have had a number of customer with the same issue and the response has been the same from HeatNGlo.
While away for a weekend trip, a family member found the gas (propane) fireplace insert in our living room to be on with full flame and two items near the fireplace where melted or scorched due to the high heat. Fireplace had been turned off, by remote control, since February 2018 and pilot light was also turned off. Family member was advised on how to shut the fireplace down completely and removed the burning items from the house. Damage is contained to approximately $300 including the cleaning of curtains, brick and other items in the living room. The installer of the insert, Country Side Stove and Chimney will be inspecting insert and chimney for any defects or damage. The insert has been installed since 2012. Upon our return home I found the batteries on the remote control to be exhausted and no display was present on the remote's digital screen. When last used, the fireplace was in manual mode (not controlled by thermostat) and the remote control was completely turned off. A previous incident also occurred in November of 2017 when I returned home from work and entered an extremely hot living room and the fireplace insert was on with full flame. No items were located near the fireplace at that time as it was during the season when we would begin to use the insert. At that time, the batteries in the remote control were also exhausted and the pilot was not turned on. During both occasions, the brick surrounded the fireplace and the living was much hotter than when we actually use the fireplace. Had I not returned home, or had not the family member checked on our home, I believe that a fire would of have started due to the intense heat generated by insert when it self-ignited while turned off. The brick surrounding the fireplace remained hot for almost 10 hours after the fire was turned off. A very dangerous condition exists for users of the fireplace insert. When my wife got up on Sunday June 1st she called me right away to inform me that our fire place insert has self ignited and no one had turned it on. I had removed the battery's from the remote which starts it to protect it from acid battery leaks. Sure enough it was lite and the pilot keep trying to relight even thou it was lit. I had to shut off the gas main valve to stop the flame. Then switch off the pilot lighting switch to stop it from trying to ignite it. I felt this was very unsafe condition and called the installer (Schagrin Gas Company Middletown Delaware) and they told me it wasn't there problem it was the manufactures issue. I then call them (Mendota at Johnson Gas Appliance Company [REDACTED] - Cedar Rapids, IA 52405 and informed them of the unsafe condition that their Mendota Model FV44i-Mod ignited by it self and I was lucky we were home at the time. They informed me that they would have a technician check it out. They sent a Schagrin expert to investigate the issue and he found the receiver for the mother board failed and when he turned on the pilot switch it tried to lite without the remote. He had to get a new receiver for the mother board and replace it. I feel this is a very unsafe condition for public use and the Mondota company who design and built it should recall and reprogram the unit to shut off when a receiver or mother board failure not lite off. We were so lucky to be home when this happen.
We just recently purchased a home with an existing Lennox Fireplace that has a battery-operated ignition system. When we put the batteries in the system they pop and start to leak.
The caller stated that whenever the gas fireplace is turned on, approximately 20-25 mins later the CO detector comes on. He turns it off to prevent CO poisoning. This has been ongoing for 3-4 years. He has not contacted the manufacturer as yet. He feels that this gas fireplace (free standing ) is a health hazard.
